Manages, facilitates, andcoordinates all aspects of patient care (i.e. historytaking, clinical assessment, diagnosis, treatment and evaluation of care) with direct interaction with physicians and surgeons.
Essential Responsibilities and Duties
•Assesses, diagnoses, plans, implements, and evaluates treatment/interventions and care.
•Prescribes and reviews medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and international standards and protocols, and within scope of practice.
•Identifies post-transplant complications both short and long term, confers with physicians for intervention and communicate recipient status with physician, and other involved healthcare professionals.
•Works with patients in order to support compliance with and adherence to prescribed treatments and provides information and advices on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
•Monitors surgical and medical complications, rejection, infections i.e. CMV, HSV, wound and nosocomial infection, medication side effects, long term complication (i.e. recurrent disease, chronic rejection).
•Prioritizes health problems and intervenes appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
•Identifies educational, psychological, and economical needs of recipient /family. Educates recipient and family on care and responsibilities after transplant; vital signs monitoring, diet and activity, required follow up visits/labs, reporting abnormalities to transplant center, signs and symptoms of infection/rejection, explanation of required procedures post-transplant (e.g. biopsy).
•Manages co-morbidities and non-infectious complications (Ex. Optimizing HTN, DM medications, caloric intake, and increase physical activities).
•Collaborates with nursing staff and social workers to establish care plan, provides comprehensive documentation of progress and education, promotes prevention of long-term complications and improves the quality of life (involve social support, facilitate coping with ongoing education, counselling and follow ups).
•Coordinates discharge planning, demonstrates ability to complete discharge teaching and documentation.
•Attends clinical rounds, other related meetings and participating in committees as assigned.
•Participates and contributes to the research activities of the centre including involvement in data collection, dissemination and reporting.
•Participates in the professional development and training of clinical transplant coordinators.
•Follows and monitors highest standards of professional and ethical codes of conduct.
•Maintains and collects clear documentations pertaining patient s data.
•Follows all Hospital related policies and procedures.
•Participates in self and other s education, training and development, as applicable.
•Performs other related duties as assigned.
Education
Master s or Bachelor’s Degree in Nursing or Registered Nurse with country specificpost registration program is required.
Experience Required
Four(4) years of experience in Nursing including two(2) years of experience in specialty area with Master s Degree or six(6) years of with Bachelor s Degree isrequired.
Other Requirements(Certificates)
•Current Registered Nurse Licensure from country of origin and Saudi Commission for Health Specialties
Licensure is required.
• Teaching qualifications is preferred.
