The Program is a Project Team member based at the MOH, that will be in charge to coordinate the activities between the MOH (specifically Committee) and Treating and/or Referring Centers. The Program Coordinator shall report the project progress and key milestones to the program manager. The Program Coordinator is responsible to:

Review submitted “Patient Approval Form” (PAF) forms for completion and data sufficiency, contact the requester in case of missing/unclear/inconsistent information and upload, if needed, the completed forms to the eHealth link platform for the committee review.
* Ensure respecting defined timeframe for PAF submission process defined as 2 hours.
* Reach out to patients/caregivers to secure written consent and introduce the program in full details
* Refer patient to the nearest treating center in the case where the patient was referred through a Referral center
* Hand complete patient’s file to the Care Coach/Care Coordinator
* Liaise with the Care Coach and Care Coordinator to ensure file transfer to the treating center and journey initiation
* Report any potential adverse events, product quality issues or other perceived safety risks.
* Refer specific medical queries or issues to other healthcare professionals in accordance with local guidelines or company protocols where appropriate.
* Communicate with and develops relationships with other healthcare entities/organizations as deemed needed for the Program and within their scopes.
* Establish a compassionate environment by providing emotional and psychological support to patients/caregivers.
* Report the client’s condition and any changes to the program Manager and send progress notes to program Manager on frequent basis
* Accurately capture patient details following telephone interactions in the tracking software.
* Perform the necessary remote and onsite monitoring of the data uploaded into the platform in accordance to a pre-set plan detailed in the project management plan.
