Duties and Responsibilities:  5.1 Making the initial visit evaluation as applicable and re-evaluating eachpatient’s nursing needs on regular basis. 5.2 Participates in discharge planning of all identified patients for the Home Health Care. 5.3 Providing nursing services, treatments and diagnostic procedure for theHome Health Care patient. 5.4 Developing and implementing the treatment plan for each patient under the direction of their physician. 5.5 Demonstrates leadership and management skills when acting as the Team Leader position. Team Leader is first level of administration assigned to the unit in the absence of the Head Nurse. The following duties and responsibilities are as follows (but not limited to): 5.5.1 Provides professional leadership and acts as a role model and mentor to fellow nursing staff. 5.5.2 Demonstrates critical thinking and provides leadership in conflict management and problem solving. 5.5.3 Promotes and support evidence based nursing. 5.5.4 Collaborates with the interdisciplinary team to provide patient and family centered care. 5.5.5 Demonstrate a professional commitment to patients and fellow nursing staff. 5.5.6 Adopts an interdisciplinary team approach based on mutual respect and direct communication between health professionals. 5.5.7 Liaises with the Head Nurse/Nurse Supervisor to ensure that patient care load is addressed with appropriate staffing and skill mix in an environment which is safe for the patient and nurse. 5.5.8 Ensures the nursing staff works within their capacity, experience, training and scope of practice. 5.5.9 Participates as assigned on committees, working groups and other organization activities. 5.5.10 Act as a preceptor to less experienced staff as required. 5.5.11 Monitors clinical care to ensure clinical standards are met. Identifies variances and initiates corrective measures as required. 5.5.12 In collaboration with the Head Nurse, shares in the responsibility of formal evaluation of staff performance and achievement of annual performance goals. 5.5.13 Monitors and coordinate effective and efficient utilization of human resources consumables and equipments. 5.5.14 The Team Leader will be expected to ensure compliance with the Policy and Procedure of Hospital. 5.6 Observing and reporting symptoms, reaction to treatments, drugs and changes in the patient’s physical or emotional condition. 5.7 Facilitates the issuing of the required equipment and supplies for HHC patients. 5.8 Facilitates open channels of communication with all HHC team members. 5.9 Maintaining clinical and progress notes for each patient receiving care and providing progress reports to their physician. 5.10 Counseling the patient and their family in meeting nursing and related needs. 5.11 Documents the type of wounds, location and the condition of wounds. 5.12 Keeps records and updates documentation of patients in the Home Health Care. 5.13 Maintaining confidentiality of patient, thus maintaining patient rights. 5.14 Submitting required documentation in timely manner. 5.15 Organizes patient visit schedule per individual patient discharge plan and monitor its implementation. 5.16 Provides feedback to the health team members. 5.17 Participates in the staff development. 5.18 Participates in Home Health Care Team meetings and advise the Director of Nursing as required. 5.19 Participates in CQI&PS;activities. 5.20 Is responsible for keeping updated in Infection Control, Fire and Safety and Disaster on a yearly basis. 5.21 Is responsible for keeping Cardio-Pulmonary Resuscitation certification and updated on a two year basis. (Saudi Heart Association Recommendation) 5.22 Promotes health education to patient and their family regarding patient status. 5.23 Privilege to handle Narcotic and Controlled Drugs.

Good interpersonal and communication skills.
Knowledgeable in Basic Computer (Microsoft Office).
