The incumbent will be responsible to ensure clinicians’ documentation in electronic medical records is in line with company’s medical documentation best practice guidelines, perform analysis of reports and conduct awareness/training/campaigns to improve medical documentation.
Evaluation of Clinical Documentation Quality
Conduct periodic review of medical records based on predefined documentation quality criteria to assess adherence of clinicians to company’s documentation best practice guidelines.
Examine, and evaluate content of medical records documentation and notify physicians about noncompliance.
Review utilization clinical forms in paper medical records or EMR templates whether they are used in coherence with care rendered
Liaise between clinical coding team & physicians regards to physician query process that is connected with quality of documentation.
Maintain up-to-date knowledge with clinical documentation guidelines, coordinate implementation of clinical documentation improvement programs by Health Information Management department.
Reporting
Analysis documentation review performance metrics and prepare physician score card for the review of company’s committee, Accreditation Team, Physician Leads, Coding Team.
Proactively involve in documentation improvement activities and provide one to one, classroom and online support for clinicians to ensure appropriate documentations are made in patients files.
Communication, Collaboration
Collaborate and communicate with Clinical Staff to clarify inconsistent, doubtful, and non-specific information in medical records.
Conduct follow-up reviews with relevant Clinical staff to ensure files are completed, information is accurate, and queries are answered.
Guide and instruct Medical Staff on documenting medical records to adequately reflect patient treatment, diagnosis, and procedure.
Patient Safety Commitment
Ensure that the aspects of patient safety, risk management, and quality improvement are embedded in all the processes under are of responsibility.
Foster a culture of safety, where staff feels encouraged to raise concerns and report incidents without fear of disciplinary procedure.
To develop a culture of learning and encourage staff to learn from defects. Learning should lead to continuous process improvement.
To collaborate with other leaders for Patient safety.

Education Requirements
Bachelor’s degree in Health Information Management or a similar stream.
Certification, license or training Requirements
CDIP (AHIMA) or equivalent certifications
Experience & Knowledge
Knowledge in EMR applications.
6 years of total experience with 2 years of experience in clinical documentation improvement and/or associated activities
